Huntington Woods Care & Rehabilitation Center, a beautiful skilled nursing and rehabilitation center in Westlake, OH, is currently seeking a Social Services Designee to join our dynamic management team. The Social Services Department is responsible for managing and/or assisting with various resident services including the resident waiver program, PASSAR requirements, Level of Care (LOC) and Medicaid eligibility requirements as well as discharge coordination, care conferences and assisting with quality assurance for resident services.
